Attached to this post is a Word document that was developed to help manage the 140 Questions that we get to choose from for our Systems Portfolio. This, like the SP, is a living document--it started when Ft. Hays Univ. in Kansas entered the questions, Dodge City Comm. Coll. in Kansas suggested some columns and additions, and Evangel Univ. in Missouri added the last column at the "Crafting Your Systems Portfolio" workshop last week. We wanted to share it with other AQIPers, hoping you might find it useful, or it might inspire some other ideas for organizing the information for the questions.
This is a 23-page legal-sized document. All 140 questions are included, but as you know, we don't have to answer them all. We hope this tool makes it easier to decide which ones to choose, and to point out opportunities for future improvement and give us ideas for future Action Projects.
At the top of each category, the minimum number of questions to answer is listed. You'll also notice that the last (P)rocess question is bolded--the reason is that if you choose not to answer and (R)esults or (I)mprovement questions, you are required to answer the final (P)rocess question.
There are columns for
1)the question;
2)the Processes or Practices you identify that are relevant to this question;
3)the Evidence/Data Source used to answer this question; and
4)which of the HLC five Criteria this question indexes to.
We intend to give this document to each of our team leaders--they will be asked to meet with their teams and complete as many of the middle 2 columns as possible. After it is filled out, we can decide which questions to pursue in depth. (The last column is intended to be a cross-reference for creating the document for Indexing the Categories to the Criteria, and will mainly be used by our writing team). We feel the Evidence/Data Source column is important because it focuses our attention on collecting evidence of our performance, AND because as we continue to update the portfolio, we will know where the documentation came from and where to go to get updates on performance.
If anyone has suggestions or other ideas for this tool, we'd be glad to hear them. Thanks to all who contributed!
